Kini manusia hidup dalam sebuah kota yang dikelilingi tembok tinggi yang menjadi perlindungan mereka dari serbuan bangsa vampir yang masih berkeliaran. Salah satu dari sisa-sisa manusia ini adalah Ivan Isaacs (Paul Bettany), sang Pendeta (Priest), yang telah bersumpah untuk tetap bertahan di dalam kota.

Suatu ketika, Priest terpaksa melanggar sumpah yang telah ia ucapkan. Ia tak mungkin membiarkan keponakannya hilang begitu saja. Keponakan Priest diculik bangsa vampir yang dipimpin oleh Black Hat (Karl Urban), mantan pendeta yang memilih berpihak pada vampir. Priest harus cepat karena kalau ia menunggu terlalu lama maka keponakannya bakal berubah menjadi vampir dan tak ada lagi harapan.

Untungnya Priest tak sendiri. Ada Hicks (Cam Gigandet), sheriff yang juga adalah kekasih keponakan Priest. Selain Hicks, masih ada seorang pendeta wanita (Maggie Q) yang punya keahlian bela diri sangat tinggi. Meski mereka bertiga adalah prajurit yang tangguh namun usaha penyelamatan ini tetaplah bukan usaha yang mudah. Black Hat dan pasukannya tak akan begitu saja mengalah.
